Healthy Directions Program
- Wellness preventive visits are by calendar year, not year-to-year, i.e., January - December.
- A primary care physician, or PCP, includes those who practice internal medicine, are family or general practitioners, or are OB-GYNs. You can call 1-877-571-9867 or visit myuhc.com for help finding an in-network PCP.
- A wellness visit is considered preventive care. If during a visit with a PCP items such as high blood pressure, stomach problems, sore throat, etc., are discussed, the visit now then becomes a diagnostic visit and will no longer fall under a wellness visit. When the visit becomes diagnostic, you will be required to pay a co-pay.
- It is ultimately between the PCP and the employee on what blood work needs to be completed; however, the only blood work that will be covered under wellness, paid at 100%, is detailed on form C of the packet.
Reminders:
- Packets can be found on the Employee Information Center (EIC)
- PCP visits must be completed by September 30, 2019 and packet forms A, B and C submitted to TriHealth by October 15, 2019
- Remember to sign packet forms A and B; the PCP needs to complete and sign form C
- It is your responsibility to fax or email packet forms A, B and C, not the PCP.
- Keep copies for your records
- Once paperwork is received by TriHealth, they will send a confirmation email
